change of status from F2 to H4

Hi,

 

i am right now on F1 (OPT extension) ,my opt is expiring in decemeber 2013 ,and i have my H1 approved from 2013 october to 2016 so from october 1st i will be changing my status to H1.

 

but right now my wife is on F2 so i want to change her status from F2 to H4 ,

i just want to know the check list for COS from F2 to H4 along with I539 form,

i am not able to find it anywhere on the google please help me...

 

 

and on I539 form i see current nonimmigrant status expires on field ,

so my question is do i need to put dec 2013 in which my opt expires

or

october 1sr from which my status changes to H1 which means my F1 goes off which means F2 is not valid

 

so which one shall i put there?
