Company Ready to file GC. Missing Document - OPT approval notice . please help

Hi ,

 

       I am on H1B since Oct 2011. My company is ready to file GC and to order to start process they asked for Visa Approval Notice like F1 approval , OPT approval , H1B approval Notice. I have OPT EAD card but I lost OPT approval Notice. Is there a way to get a duplicate copy of OPT approval notice from USCIS while I am no longer on F1-OPT and already transferred my status.

 

Please help.

Not sure if the rules changed recently, but in my case a few years back the OPT card was enough.
