H4 Visa and NOC document - Advice

Hi,

My wife is working in government hospital (temporary employee) and now ready to resign and I have applied for her H4 visa.

Question :

1. Should she get NOC before taking her H4 visa stamping ( July 3 rd 2013)

2. She is doctor (M.B.B.S) is there any other document other than H4 regular document ?

Can you please advice me on this.
