H4 Renewal process for stamping

Hello everyone !

I am on H1B and wife on H4. Our VISAs expired on Sept 2012 and we have since renewed our VISAs. Both of us have received the I-797 approval notices. My wife left for India on 12 April, 2013 and plans to return in May/June 2013. Her passport has the exprired H4 stamp, thus she has to visit US Consulate for her H4 Renewal Stamping. My question is, can I get some help regarding the renewal process for stamping.

(1) is the first step filling out the DS-160 form ?

(2) Then what will be the next step ?

Also, I think the VFS website has stopped working. I did find its substitute (if I am correct !!), whcih is http://www.**********.html. This website is really confusing and I am having very hard time just to find the list of documents required for VISA interview.

Also, I am resident of Maharashtra state. For my first time H1B stamping, my wife H4 stamping (first time) we visited to US Consulate in Delhi because, that consulate does not require to submit visa related documents 3 business prior. Now since this VFS does not work, is there any changes in this method ?
