Need help in filling I94 for first time

Hi I am travelling to US for first time.

I am confused in filling names in I94. seems like this will be used for License and other stuffs.The details is share below.

My Passport:

SurName:

Given Name: vvv kkk sss

My Visa Stamping Page:

Given Name: vvv kkk sss

First Name: FNU

My I797B notice page:

Name: vvv kkk sss, LNU

What should i fill in family name and first name where i should not find any difficulties in filling SSN and getting Driver license.

Please please can some one guide me in this regards.
