L2 to H1 trasnfer

Hi

I came to USA in 2009 with H1b which was valid till 2010 September. But in 2009 end I have changed my status to L2 and started working in 2010 beginning itself after getting teh EAD card. So I didnt renew or change the visa to H1B again. But now I am planning to start a new job with H1b and want to know the procedure to move to the H1b again.

1. Do I need to use the same old H1B and re-enable?

2. Do I need to file for a new H1B in 2013 and also need to go through the cap process again?

3. If I get the H1B, do I need to wait till October 2013 to start working with the new H1B?

4. How long will be the new H1B valid for? total 3 years or remaining of the previous H1B period? (I had workded in my previous H1B for only few months.)
