Need Urgent help regarding H4 Visa

i need to apply for my wife H4 visa. I need clarification on few points.

1) do i need to add my spouse name on my passport before she appears for the interview

2) does she needs to change her last name on her passport as her last name is changed after marriage.

3) which bank accepts the visa fee. i heard HDFC is no longer accepting visa fee.

any help will be helpful.

No Need But you need to submit the Notarized copy of your Marriage Certificate
