Filing for H1B from OPT extn

I'm already in USA and my employer is filing for my H1B from my current F1- OPT extn stage. My wife is in India.

1) Can I apply for her H4 visa from USA?

2) Can I apply for her H4 visa after getting my H1B confirmed in premium processing?

3) Can I apply for her name change after she comes to USA.

4) Also does my employer have to file I-797 for her in my case because Im already in USA

She can apply for her H4 AFTER your H1 is approved and you can be in US while she applies. Your employer doesn't need to file anything for her. Your 797 should be in your hand.
