Old & New Sevis - First time OPT

Hello,

My sevis got terminated from my previous school and I crossed the border at mexico with a fresh i20 and new sevis from ***. Given this and being on a CPT for the last 6 months, I am now to apply for my OPT.

My quoestion is do I need to put all my previous i20's with my old sevis number on them??
