F2 to H4....Approved but not recieved approval notice...what to do next?...plz help

Hi,

I was on OPT when my employer filed for my h1b on may 2011. Also filed h4 for my wife at the same time, who was on F2.

My h1b and my wife's h4 have been approved.But we have not received any approval notice for my wife's h4.

A notice was sent to the attorney from USCIS saying that h4 has been approved. The notice also contains the reciept number.

Now should I apply for a duplicate approval notice (I 824) ?

Will this I 824 application also issue a duplicate I94.

Is the new I 94 issued with h4 needed at all, as she already has an I94 when she entered the US on F2.
