H4 Visa stamp contains Old Name while new passport is having new Name

My wife got her H4 stamping in Chennai consulate during Aug-2011 (which is valid upto Apr-2014) with her old name. We had to update her passport address & name due to getting new passport for our son.

So, she is having the new name in her new passport. But, H4 stamping is available in her old passport with OLD name. Can we travel to USA along with same visa or do we need to do stamping again?
