Councilor approved visa and took passport on oct 4, in hyderabad India but visa status shows “VISA REFUSED “ and Passport still with us embassy with out any update since last 3 weeks

I attended the for H1b visa on oct 4, 2021 in hyderabad India and councilor took the passport without giving me any letter or colored slip. But, when I check the visa status it shows “ VISA REFUSED” and upon checking the passport tracking it says passport is still with us embassy. It’s been 3 weeks for now but there is no update on my visa status or passport collection.
