H4 visa getting expired by 31st July but I94 provided till 23rd Oct 2020

Visa is for entry to USA and i94 dictates for how long one can stay in US. Your wife can stay as long as i94 is valid. If she leaves US after 21st of July , she will need a new visa to enter united states.

Do i need to file h4 extension ?

She can stay till 23rd October 2020 based on I-94. If she travels out of country after July 31st she will need a new visa stamp.
