Documents needed for H4 EXTENSION

Hi,

I would like to know what documents are needed to extended my H4 status , Is there a separate form for bio metrics ? and should I send the money order separately for bio metrics ? please share any official link

G-1145
Cover letter
I-539 new form (No separate form for biometrics)
check   $370 and $85 (separate checks)
I-94
marriage certificate
previous and current visa
passport pages
driver license
Spouse H1 Copy
Spouse employer letter

Spouse I-140
Spouse H1 visa
Spouse I-94
Spouse passport pages
Spouse pay stubs
Spouse W2

Once you ship the application USCIS will send you the receipt and also a biometrics appointment location, time and date.
