ppatel115 Posted March 4, 2019 Report Share Posted March 4, 2019 Hi, I am F-1 student applying for initial OPT, there is a section in form i-765 (Part 2 Box 2), in which we have to fill out the other names used, so in my case I have used only 1 name while in the United States(Passport, SSN, State ID and Masters Transcripts) but while in India, I have different name in 10th, 12th and b.tech transcripts and degree. So should I fill out all the names I have used in India or just filling out the name on my Passport is enough. Awaiting your response. Regards Pooja Link to comment
