Things to make sure while PERM is applied

Hi there,

My company is filing my PERM and asked me to review the PERM application.
I need help for couple of things below:

1. Apart from my basic personal information like date of birth, passport number, FN,LN etc., what are the other things I need to make sure that are correct. 
2. PERM application has a space to mention past three years job experience. But part of my GC's job requirements match with my previous job that I did five years back.  Should the PERM application also included this five years back job experience along with last three years job experience?

3. Should I include the experience letters along with my PERM?

I am trying to get some help to provide right information in the PERM application and do my best to avoid RFEs or rejections.
