H4 EAD with h1 extension receipt

Hi, 
I have a situation and need your help. My employer have filed for my h1 extension in june and received RFE. They are planning on replying to RFe by mid-end nov. My wife is currently on H1 and her h1 expires nov 10. Her employer is not extending her h1. 
We want to file for h4 COS and H4 EAD. (Based on above conversation it seems we can file h4 COS and H4 ead with h1 receipt) 

My question is
Where should I send the H4 COS and H4 EAD package?
Option 1: lewisville tx (which is recommended for h4 ead mostly with h1 approved notice)
Option 2: where my h1 was applied. Think its Nebraska. 
Do I need to send all documents in one envelope.
