Re-Entry Permit form and fee question

On the Form I-131 For Re-Entry Permit for a green card holder (the Category on the card itself says F41), how shall I answer the following questions on the form:

1. Class of Admission

2. On Part 2. Application Type I check the box "a. I am a permanent resident or conditional resident of the US, and I am applying for a reentry permit". I do NOT need to complete questions #1 - #5 (Name, Date of birth, Country of Birth, etc.) below checkbox "f", correct ?

3. Is it correct that the total fee is $360 + $85 (for biometric fee) = $445 ?
