H4 ead application question 11

Hi experts 

 am filling I 765 application for h4 ead and the question on application has this...

Have you ever had work authorization before from uscis?

for this I think I am answering yes as I had opt and opt extension before and I mention opt as work period.

should I mention H1b ( my guess is h1b is not employment authorization or ead. )

Also I had my H1b approved oct 2014 and revoked dec 2014

please advise .

Since you had OPT work authorization earlier, Please mention the latest dates there and attach all the copies. Don't mention the H1-B employment.

H1 is employment authorization. Did you get paid while in H1?

And NEVER try to hide things. They have access to your file, and can see if somebody is trying to hide things. It would cause lots of problems.

Thank you very much for the response. Can I know the required documents. I mean do I need to submit my OPT card front and back r just front pic is fine. Do I need to submit my OPT extension card details also.

You need to submit all the OPT cards from Day 1-END and enter the latest approval date of the OPT card on the I-765 form. Include front and back pics of OPT card.

I lost my OPT extension card I have front pic of the opt extension card. Is there a way for me to get it know. What happens to my H4 ead application if I didnt submit the back pic of the opt extension card.
