can h1 travel to India leaving h4 in US?

Hi

I got my I-797 approval from company A during Januray 2017 and its valid unti 2019.

From company A filed for H4 and its not yet approved.

In this scenario, can I travel to India leaving H4 in US?

I've to go for stamping in India and In DS-160 I've to mention spouse is in US.

Will this affect my stamping?

Yes you should be ok to travel

The problem is the stamping. What if there are issues? What if you get a 221?

Technically, a person is only in valid H4 status of the H1 person is in valid status. Short-term travel abroad is generally not an issue, but with stamping, things can get dicy. Talk with a lawyer.
