How to take a single appointment for H1B and H4 visa stamps?

First you need to fill separate DS-160's. Then you can go to the scheduling website (us travel docs), fill your (H-1) information first and then it gives an option to add a dependent (H-4). At that time you can enter H-4 information and schedule the appointment. It will give you one slot for both of you.
