Is sponsor held liable for any bills that result from medical care they may receive when coming to the USA? What happens if parent-in-laws are not able to pay the medical bills? Would I be responsible?

Have them get adequate insurance. Are you going to tell the Ambulance personnel that you will not be responsible. ? Are you going to leave them outside the ER? If they have serious medical issues they should reconsider the visit.
