USCIC case status update

Responded to an RFE and it got updated on Nov 18th. USCIS case status for my H1B application states that they will start working on my case again as they have received response to the RFE.

I have heard that the case status updates in the USCIS website is not updated properly/regularly.

Q1. Does it often happen that USCIS website status still shows an old status and the case may have proceeded and the employer may have got a more recent update? 

Q2. What are the current (as of 2016/17) status updates steps after USCIS has received an RFE response?

Q3. Is there a difference between updates in the case status update page and the online account page?

I just have the case number. I have not set up a USCIS account. I think the company may have done that (?)
