F2 to H1B Approved with old i94. Am I legal to work now?

Hello Sir,

I am on F2 dependent Visa and applied for H1B in April 2016, it got selected in lottery and H1B got approved on Nov 2016, I have couple of questions which are brothering me. your help is much appreciated,

1.       My H1B request of i797 got approved on 17th Nov 2016 with my old i94.(which is valid from Dec 4th 2015 to July 29th 2016) on July 29th 2016 (During my H1 process pending I exited to India and I am back into USA on Aug 18th 2016 with new i94.) 
 
2.       In approved I-797A  it says my request for change of status is approved.
 
3.       did COS happened or not? Am I on H1B status from Nov 17th, 2016 or my status is still F2?
 
4.       Am I legal to start working on H1 now? is working with old I-94 on H1 is legal? Or Should I go for stamping outside US ?
 
5.       Now my present employer is not helpful and he need me to do H1 transfer.
 
6.       Is it possible to do H1 transfer without have any payroll?

It would be ideal if you let me realize what are the alternatives that I have and kindly do the needful
