Help need in filling Profile for Canada H1b Visa Stamping

I am trying to create a profile to book an appointment for H1B stamping in Canada ,

I got my F1 Visa in 2012 ,which is valid till 2017. My first H1B is valid from 2015-2016. I got my extension recently which is valid from 2016 to 2019. Later I applied for amendment whose dates are same as my extension dates. 

Can you please let me know the details need to be filled for the below attached Image

I think you have to provide F1 stamping details since that is the most recent visa.
