My wife and son(3 months old) going for stamping for 1st time. does they need separate DS160 and appointments?. does my son should go along with my wife for stamping?

Situation:

I'm having H1 b valid till Dec 2019. I'm in USA now. My wife and son(3 months old) back in India. They are going for stamping for the 1st time. 

Questions:

1. Does my wife and son(3 months old) require  separate DS 160 and separate appointments?

2. Does my son also should go along with my wife for stamping?
