Is it possible to get L1 extension denied without any RFE?

Of course it is possible. They can always deny without an RFE. An RFE is usually issued if there is some doubt.

Nobody here works for USCIS, so we simply don't know, we can't know.

any visas can be denied directly without any RFE
we will not know the percentage for denial, as said we cant
your employer may know how many applications are denied in your company, they can give an idea
