Documents required for 24 months opt extension

Hello Experts,

                         I am currently on 12 months STEM OPT EAD and want to apply for 24 MONTHS STEM OPT EAD, what all documents do i need to submit at the time of extension and do i need to provide pay rolls/pay stubs or just F1 OPT I-20, I-9, i983 and i765 be enough?

what are the frequent causes for OPT EXTENSION RFE's, ?

I have changed 2 employers during the 12 month OPT period and my current employer is the 3rd employer.There was no gap in between me changing employers and I have not been unemployed during the 12 months. I have updated the university regarding my change of employments and new i-20 has been given to me.

Is there anything I need to be careful while applying for OPT extension?
