Request for Initial Evidence for STEM Extension

I got second RFE from USCIS asking for myI983 form itself for STEM application .Now,I've found that by mistake my employer had pasted another employee's name for a question in the page 3 of i983. What am I supposed to do in this scenario? Is there a chance for denial? Why have I got this RFE in particular? Thanking you in advance
