spouse's full name (include maiden name) DS160

Hi,

Can someone please help me in figuring out how does the name needs to be filled in for spouse in DS 160 (Family Information Section)

Wife's name  before marriage in passport :

Surname: Mishra

Given Name: Sunita 
Wife's name after marriage in passport :

Surname: Tiwari

Given Name: Sunita 

The section says please enter Spouse's Full Name (include Maiden Name). And there is a Note that says :Enter current spouse information.

So should the name be filled in as : Last Name : Mishra or Tiwari ?
First Name : Sunita 

Need an early response please.
