Documents needed for EAD renewal

I have an EAD card that is expiring in Sept 2011. Can someone please tell me what is the procedure to renew my EAD.

I would also like to know:

- How many days before the expiration date should I send my renewal application.

- All the documents needed to file the EAD renewal.

- Where do I send my application.

- What is the fee.

- How long does it take.

- Can I file for Advance Payroll to travel outside USA along with my renewal application.
