Mother's name different on passport - Visitor Visa - US

Hi,

I would like to sponsor my mother for visitor/tourist visa, but my mother's first name is different on my passport than on her passport - she is also knows with that first name but its not documented properly.

for example: on my passport (mother's name as - eshwari) and on my mother's passport (eshwaramma), last name is same.

do I need to take any affidavits or if I just mention the other name as alias name in DS form?

Pls advice on this.
