Information: H4 - Going for stamping alone

My wife had to travel to India for a short visit and planning to go for stamping. We had few doubts while filling up the DS-160 and the information while scheduling an appointment especially about principal applicant information. We googled about it and filled out the information based on the suggestions on the forums(including this).

However, after taking the appointment coincidentally I came across other forums suggesting a completely different view about principal applicant information. So we called up the US VISA help line (1 310 616 5424) and here is what they informed us. Please note that I called them twice asking about the information just to make sure I get the consistent answers. So both the time, here is what was suggested...

DS-160:

-------

Principal applicant surname : My last name

Principal applicant given name : My first name

Application Receipt/Petition Number: My I797 receipt number.

Scheduling appointment:(https://cgifederal.secure.force.com)

-----------------------

Principle Applicant's Name: My name

Principle Applicant's Visa Class: My VISA class (H1B)

PETITIONER

Name: My company name

Start Date: my start date on I797 (it is same as my wife's 797)

Receipt Number: my receipt number

Expiration Date: my end date on I797 (it is same as my wife's 797)

Should the principal applicant be always me? The helpline i called they said "unfortunately we will not be able to suggest how form needs to be filled"

I am assisting my wife to fill out DS-160 to renew H4 visa,i have few questions related to that and scheduling the interview date, below are the points/notes & questions listed, kindly help us.

 

Notes

 

i)  My wife is currently in India and we both have our petition till mid November 2017 & I am currently in United States so she is the only one who is going for Interview, I also have my i140 approved

ii) She has i797a of H4.

 

Questions

 

1. On DS-160 form under Travel information section there is a 'Principal Applicant Name' & 'Application Receipt/Petition Number', whose name should be filled out in that field ? I guess its the H1 holder name i.e. Me & My application receipt number, Am i correct?       kindly advise

 

2. While scheduling the Interview Date on "https://cgifederal.secure.force.com/updatedata" Under Mailing Address section there is "Principle applicant's Name" & "Principle Applicant's Visa Class", whose name should be in that filed and what visa class ?

    should it be my name or her name and should it be her visa class? 

 

per your suggestion it seems principle applicant should be me the H1 holders name & my petition number on the scheduling URL too ((https://cgifederal.secure.force.com) ) 

Also the petitioner should be my employer correct?
