question about Ds-160

I am filling out my Ds-160 form for my H1b, and planning to got to india in nov.

1)It has question as -U.S. Point of Contact Information:

should it be my employer name and address or my husband’s? I stay in US with my husband who is also on h1.

2) for a question-

Have you belonged to, contributed to, or worked for any professional, social, or charitable organization?

-Can I mention my current and previous employer details here? or shall i say NO.

I havent worked for any social/charitable organization.

Please suggest.
