H1B extension denied: does a new H1B come under the Quota

I was in the USA for 3 1/2 years and my 4th year extension got denied. I immediately came to India within 10 days as I cannot apply for MTR as my project is over with the client at the same time. My employer is trying to get all the paper work together and the client letter to apply for a brand new H1B. Couple of questions:

1) Does my new H1B come under the 65,000 quota limit for 2012?

2) Do I need a client letter to apply for the new H1B?

3) If I apply for a new H1B, would the be for a new 6 years or would it be only for the remaining 2 1/2 years from my prior H1B?

A quick response would be greatly appreciated.

Since you do not mention the most important fact which is the date you left, it is difficult to give you a sensible reply.
