Can any one please share a link to the sample copy of approved LC and I-140 documents?

I would like to see how the approved PERM and I-140 look like.

sample copy is nothing but a dummy copy.Sample copies will never have any critical data in it.

Look at the form 9089 online. They certify on the same form and send it to the employer with a cover letter.
