H4 Extension RFE

RFE:  We have received an RFE for our H-4 extension and they were asking for latest I-94 document and 

Additionally, please submit all I-94s issued to you subsequent to your initial entry and associated with any approved requests for change of status or extensions of stay

Question 1: What are all the documents we need to submit for this RFE please?

Question 2: If all previous I-94s need to be submitted, we do not have one previous I-94 document copy for the travel I made in 2015, how do we obtain previous I-94 copy?

FYI: We are able to obtain the latest from https://i94.cbp.dhs.gov/I94/#/home and we have all I-94 copies of change of status and extension as well.
