I485 - Indian Embassy Birth Certificate

Hello Team,

I do not have Birth certificate and local authorities in my town also do not have such a old record (1976). I requested Indian Embassy to share the Birth Certificate with me and I received one from them.

The question is.

1. Is the Indian Embassy provided Birth Certificate is valid to file I -485/Green card?

2. What is the alternate option I have for this?   

3. On couple of website I read that affidavit from close relative is accepted. Can someone share the format of this affidavit?
