I-140 withdrawn after 180 days and cap-exemption

My I-140 approved in Feb 2011 and moved to India in July 2016(H1B has 3 months left of max 6 years) and left company A. Company A withdrawn I-140 in Dec 2016. I rejoined company A in Nov 2019. Can you please help on my options to get H1B cap-exemption in future

 

1.       I was with company more than 180 days after I-140 approval? I do not see status on USCIS. Company says I-140 was withdrawn in 2016. Is it really mean of withdrawn. Can I use I-140 to get extension with any other company B?

2.       Can I get H1B cap-exemption on withdrawn I-140 beyond 6 years? Company A or Company B

3.       Company A says we will reinitiate the I-140, what are changes of getting approved?

4.       Can I start new PERM and I-140 from India?
