PERM is still showing CERTIFIED in DOL, Not Sure I 140 raised?

Dear Sir or Madam,

 

Old PERM from old employer is still showing CERTIFIED in DOL performance data, But I am not sure whether I-140 is raised or not for me since i left that organization during that time . Is there any way we can know the PERM has been used and I 140 raised ? Appreciate response.
