DS 160 Past Employment details

Hello,

While filling DS 160, we need to specify the past employment details for 5 years. I have been a consultant for 6 years and the last 7 months I am full time. So, the last 6 years details should be Consultant company (Employer) name and address or the clients name and address for whom I worked for?

Please clarify.

It should be the name of your employer on your pay checks or W-2 when you were a consultant.
