No Birth Certificate

I am in I-140 filing stage for my GC process and i came to know from my attorney that my case can be cross chargeability as she was born in UAE (Year : 1988).  We don't have birth certificate or any other document related to her birth, her parents came back to India few years after her birth. 

Only evidence i have regarding her birth is baby Passport issued by Indian Embassy with her Birth Place details and her name is written on parents passport. My attorney suggested to file I-485 with two affidavits in lieu of missing BC. 

I would be great help if anyone can share affidavit template used and got approval. Also do i need to submit her parents passport as well as evidence.  

What would be chances of approval with by just submitting the affidavits
