I94 with two employeer and its validity

I am H1B visa holder with I140 approved. I was working for Employer A and had I94 till Dec 2018.  Also in 2018 i transferred my H1b to Employer B and got I797A with I94 upto Jan 2021 and worked for 5 months with them.  I once again went to employer A and they applied for extension before oct 2018. For which a denial notice was sent on Dec 2018. My attorney at employee A they filed a motion to reopen (amendment) and i got a I797B for that on Jul 2019.  

I know one of my option is go back and get visa stamping.   

Now my question is

  1) Am i out of status with Employer A?.
  2)  Now I wonder what questions or problems i might face at the consulate? If so, what are my options

  3) Emp B I94 is valid for me?

   4) Using Emp B Approved petition and I94  Will i be able to transfer that H1B petition? 

   5) What happens I-140 when i go for visa stamping.  Does I need to reprocess i140 like LC, PERM and etc but retain priority date.?
