Does h1b renewal/transfer/sponsor falls in cap exemption under USCIS 2019

This might be redundant query but I request you to bear with me. As I learnt from my ex-employer that this year USCIS has changed the rules.

In year 2015 ABC company based in US (Minnesota) has sponsored me visa.

-  1st Visa Issued on Dec 2015 with 1 year Validity till Sept 2016
-  2nd Visa extended with 1 year Validity till Nov 2017 (Extended in USA)
-   As my project at Client Location (Cincinnati, OH) got end I left US on June 2017. 
-  Current Status of my visa is expired on Nov 2017
-  I stayed in US for 15 months out of 6 years

In India after working in 2 companies I joined 3rd Company XYZ and after working 6 months now my organization wants me to work at Client location in USA (NJ). My Manager is asking me for my h1b renewal.

Could you kindly help me in answering my below queries. I am very much concern as I 
- Do I need to apply only in the yearly window which is opened now by USCIS ?
- Am I not eligible for CAP exemption?
- What will be the process to renewal of my Visa extension will it be renewal/transfer/sponsor? (I apologize if I am using wrong terminology coz am not aware properly)
- My organization wants to apply in regular rather than Premium. So how many days will it take if I go for regular filing?
