Questions filling H-4 form (change of status)

Hi all,

I have few questions filling H-4 form (change of status). 

Question 1: I am on F1 OPT and stopped working one month back and with in a week from now I will be applying for change of status from F1 to H4.

i) So will there be any issues meaning am I still on valid status? (However I have valid visa till June, 2019)

ii) Will I get I94 on my H4 approval? do I have to go for stamping (my spouse has valid I94 on H1B)

Question 2: What should be the new status effective date of change in 2.a? Can I request for example January 1, 2019 or the last date of my current visa? 

 

Question 3:  In 1.a, I have to give the end date on my spouse's H1B approval? 

And In 1.b, I have to select the checkbox?

 

Question 4: In part 3, from 3a to 3f, all of them we will be empty if my spouse already has H1B approved correct?

 

 

Question 5: I am changing from F1 to H4 and worked on OPT. Currently, I am not working. So for below question I should select Yes?

Also, could you please give the checklist for applying change of status F1 to H-4 and also H4 EAD please. reg marriage certificate do I have to notarize it?

Also I have a question regarding filling H4 EAD form in Question No27. Eligibility Category

27. Eligibility Category: what should be entered here? (C26 or C35?)

Question 1:

i) No issues. Do it ASAP.

ii) You will get a I-94.

Que 2:

2.a. You can put a future date after you submit your application. If you mailed your application on Jan 10th, you can request a Jan 15th start date.

Que 3:

1.a. Spouse I-94 expiry date

1.b. Yes, you need to check that box.

Que 4:

Leave them empty

Que5:

Yes.
