H1B visa: permanently want to move India

I entered US yesterday and after coming here my family member suddenly fell ill and I want to go back to India permanently to take care. Can I directly book return tickets and travel back or I need to follow some procedure.

Sorry to hear about your situation. You can book flights and go to your home country without issue. But it is a courtesy to talk to your employer and let them know of your situation. They might be able to retain your employment in one of their locations in India.
