Returning to US

I was working from Oct 2012 to June 2018 on H1B (valid until Nov 2021) and have an i140 issued. I quit my job in July and returned to my home country, India for personal reasons and also because I wanted to take time off to recharge myself. I now want to return to work in the US. My i140 has not been revoked. My question: for an immigration perspective, what options do I have? Specifically, do I have to go through the H1b lottery again, wait until I am an year out of the country, etc?

No need to wait for 1 year, or have to go through lottery.  Your employer have to file a petition for your H1 extension based on approved I-140, and upon approval you can enter the US with your existing valid visa in passport.   If you don't have a valid visa in your passport then you have to go for stamping.
