H4 Extension Denial

Hi,

My company filed H4 extension for my dependent along with my H1 extension when my dependent were in India.  USCIS sent a "Intent of Denial" notice for my dependent case mentioned that they were in India during the petition filing, but my case approved. I am planning to go for stamping in India next month. My questions are

 1. Is there any issues to apply visa stamping for my dependent based on my Visa approval?

2. In DS-160 for dependent what i need to mention for the question "Have you ever been refused a U.S. Visa, or been refused admission to United States, or withdrawn your application for admission at the port of entry" ?   Is I-539 Extension denial is same as Visa refusal?
