L2 EAD extension while COS to H1B in progress

Hello,

I have applied for L1B extension with result pending with USCIS. Meanwhile, my spouse has applied for H1B COS, and recieved RFE (Request for additional evidence) for specialty occupation. My L1B petition and I-94 both expiring on 05-Oct-2018 while spouse's I-94 is valid till Jul 2019.

Questions I have - 

1. My spouse has valid I-94 till Jul 2019. While my application is still in processing, do we need to apply for L2 extension as well.

2. H1B COS is not approved, and his EAD is expiring on 10th Oct, 2018. Will he be able to continue to work? or we have to wait until L2 and EAD extension or H1B gets approved.

3. COS was sent with effective date of 01st Oct, will it be applicable even if his H1B is still not approved?
