Although it is not responsibility of previous employer to help you in your Green card process, typically their HR and/or department Manager can do this on good note. Since you received this new job based on experience of previous of old job, unless you changed the field completely or left previous employer on dispute, you should be able to link commonality between the two jobs and form a brief paragraph that you old employer will agree and still suitable for new employment PERM profile.

Thing you should understand is your old and new job duties are not going to change even a bit because you are applying for PERM. It is just a matter of how you project your work in words that will keep your application going. There is no mention of who from your previous employer write the statement. It is going to be just going to be on a piece of paper or letterhead of employer with wording as "This employee has worked with us from this to this date and following are the duties in brief..."
